If your operating authority (MC, FF, or MX Docket Number) has been dismissed for less than 12 months, you can reinstate or reactivate it by following these steps:

• Submit your Proof of Insurance and Form BOC-3 (Designation of Process Agent) to FMCSA.
• For Motor Carriers Only: Ensure that you have an active USDOT Number with current contact information on file. Please note that our systems will not process reinstatement requests if the USDOT Number is inactive or out of service. You can include a biennial update with your reinstatement request to update your information.
  
  

If your operating authority has been dismissed for more than one year, you must reapply for a new authority.
